How can I connect with chronic disease managers?

My company is creating a mobile application for family caregivers to streamline care and coordinate multiple caregivers within a family. In doing this it will collect data on things like medication adherence and might be used to record things like blood sugar levels or blood pressure readings that family caregivers generated. We think this data would be useful in chronic disease management and we need to talk to care coordinators, population health managers, and/or case managers about how they would use it if they had it available, what kind of data would be most useful to them, and what format it would be best for them to get it in. Our problem is finding people who can answer our questions.

You're likely to have more of an uphill battle than you think...the last thing most disease managers want is data of unknown quality, that doesn't get stored in the EHR, that creates a new requirement to look at something/ potential liability if they don't. And they're so inundated by tech stuff (and have seen a number go bankrupt) that a fair number won't even look at you unless you have $XM in the bank.

Don't start with the assumption this data will be useful to them...if you want to tap this market you instead need to ask how they will be able to monetize it more than competing alternatives.

I tend to agree with Dr. Goel. Have you thought about marketing the app to middle aged adultstaking care of an elderly family member? For instance, if there is an elderly person who needs care, the app could help the daughter who lives in the same town and is doing most of the checking, the son who lives across the country and can only call mom sometimes and the part time home health nurse record, communicate and organize their findings and concerns day by day with perhaps some resources to look up information such as what is a normal BP for that age etc. As for finding people to talk to you could volunteer with and organization like Meals on Wheals, or ride along with another volunteer and talk to the caretakers of their clients.
