What are some of the best practices when it comes to community building, outreach and management?

This is an insanely broad question - trying to do build a community and managing a community that exists are so different. If you're really needing that much guidance or have never built a community I'd highly recommend making sure you have a great advisor who gets this. I'm sure there are a ton of folks on FD:Advisors.

We are creating a community from the ground up. Mainly elementary school teacher community. With our limited experience we've been able to cover some ground but are looking for perhaps online resources of best practices etc..

I have some experience building communities in more technical audiences, but the principles are similar across audiences. Fundamentally, people are attracted to communities that provide them a sense of belonging (though attracting others with similar interests) and provides them with helpful information, either from writers or from the community. Once those key criteria are met, there are some ways to accelerate community growth, and then the "building" and "managing" become nearly indistinguishable.

However, as Lucas said above, the answer to your broad question is difficult to summarize! Hopefully at least that little bit of a tip helps.
