Resources for designing and sourcing packaging?

We're a small medical device company and we're getting ready to ship product pretty soon. I've already designed the packaging to use very few custom pieces, but we have a few things we're missing. One is a source for bulk, high-quality boxes, and the other is a good place that prints product sleeves.

http://www.thedieline.com/ has been great for getting inspiration, but right now we're pretty low volume and cost is a constraint.

Does anyone have any experience with sourcing and designing packaging?

Ellen, if the above resources don't pan out, another route you can check on is talking to a local printer with digital printing capabilities. There are a few technologies that utilize digital printing and pre-made packaging. On the soft packaging side there are products like these -http://www.landanano.com/markets/packaging and pre-converted substrates like these -http://www.mohawkconnects.com/makeready/2012/12/03/the-big-easy-pre-converted-substrates/.

The best be though is to discuss the options with a printer. They will know how to advise you based on your needs.
